Feasibility and safety of transfemoral sheathless portico aortic valve implantation.

A recent study published in the Catheterization and Cardiovascular Interventions assessed the feasibility and safety of transfemoral (TF) sheathless portico aortic valve implantation during a transcathteter aortic valve replacement (TAVR) in a single centre approach. In this study, 81 high-risk patients with severe aortic stenosis and portico valve sheathless implantations employing percutaneous femoral access, were examined. It was observed that these implants were successful in 98% of the patients. Among the participants, two suffered strokes; two acute kidney failures; four had major bleeding; eleven experienced conduction disturbances and relative pace-maker implantations; and one major vascular complication, but there were no cases of infarctions. Additionally, a 30-day mortality of 2.4% was estimated with this procedure. The results indicated that sheathless use of portico valve is feasible and safe with a good short-term prognosis. It was stated that the approach has the potential to further expand feasibility of TF TAVR to patients with severe peripheral arterial disease. However, its applicability in cases with smaller femoral access and more challenging anatomy requires more research.
